Use the interactive map below to find a provider in your area who has an executed agreement with NC HealthConnex, or view a searchable list of providers. Please note:

• You can click the magnifying glass on the left side of the map to search by healthcare organization, practice, or by address.
  • If you search by the city only, the map will zoom to that area on the map.
• Icons on the map show facility type.
• Use the drop-down menu in the upper right corner of map to filter by provider type.
• Click on a location to get more detailed information within a box that displays on the left side of the map.
