Creating a legacy of charitable impact



The mission of the CCME Foundation is to improve the health of individuals and communities. To achieve this goal, we provide charitable grants to non-profit organizations that wish to undertake improvement projects. We prioritize funding based on a project's potential to impact the community in terms of access to healthcare and related human services through the use of measurable outcomes. We welcome all applicants especially "grassroot" organizations during their early stages of development.




INTRODUCTION

Our funding organization, Constellation Quality Health (formerly CCME), was founded in 1984 to improve the quality of health care for Carolinians. That important work continues today. Constellation Quality Health has remained focused on reinvesting in our people and communities we serve to better support our mission. The CCME Foundation was created in 2019 as a separate non-profit organization to provide grants to qualified entities in the Carolinas.

Grants and funding. Because we are committed to improving health, CCME Foundation provides charitable grants to organizations in North Carolina and South Carolina seeking to improve health quality and equity, awarding grants from $5,000 up to $50,000 per organization annually. We prioritize grants based on a project's potential to improve access to care and quality of health services for individuals and communities.

How to apply: Application and Grant proposal process

To be considered, all organizations must complete our Grant Application Form. Grant applications will be accepted from April 1 through June 15 (11:59 PM) each year.

The type of projects we fund:

CCME Foundation awards grants to eligible non-profit organizations that work to improve healthcare for individuals and communities through measurable outcome improvement projects. These projects include service, demonstration, education and clinical programs producing high-impact results on healthcare outcomes. Funding is prioritized based on a project's potential impact on healthcare improvements and access for individuals and communities, particularly the uninsured and under-served population.

Award Size and Duration

Grant awards are for a one-year period, beginning August 1 through July 31 each year, with the potential for additional funding in subsequent years. Applicants are invited to apply for grants ranging from $5,000 to $50,000. Award recipients will receive a Grant Agreement which must be signed and returned before funds are released. Eligibility Requirements

Applicant organizations must operate within North Carolina and South Carolina. All organizations with current 501(c)(3) status focused on improving the health of individuals through measurable outcome goals at the community level are encouraged to apply. Examples of organizations that may apply for funding are:

  • Faith-based
  • Hospital/provider/clinic
  • Educational institution
  • Community service

Funding Priorities

Our focus is on a program that demonstrates improved health care quality with clear goals and measurable outcomes. Priority areas include programs that:

  • Improve access to health care services to the unserved and under-served
  • Hospital/provider/clinic
  • Improve understanding of health issues
  • Demonstrate improvement through measurable outcomes of stated goals

Types of programs the CCME Foundation might fund include those that:

  • Improve healthy lifestyles and quality of life
  • Increase access to culturally and linguistically appropriate care
  • Implement consumer centered care and decision making
  • Improve dissemination of education and health related information
  • Improve patient safety

CCME Foundation will not fund:

  • Reimbursable direct patient care services
  • Facility construction/remodeling of facilities
  • Lobbying
  • Fundraising activities

Proposal Deadline: When to submit your application

To be eligible for consideration, grant applications will be accepted from April 1 through June 15 (11:59 PM) each year. All eligible proposals will be reviewed, and grantees will be selected by July 31 each year. The final decisions are made by the CCME Foundation Board of Directors

The next grant year will begin on April 1 in the coming year.

Progress Report: How to report your progress

Awardees must complete a Progress Report on a semi-annual basis. Access to the report will be provided by the Foundation and must include the following:

  • Project goals, how they were measured, and an evaluation of your performance against them
  • Any identified deviations from your project plan
  • Any identified barriers to completing the project
  • A detailed financial report in the format described on the reporting form
